REGISTRATION

If you are registering a student who is new to Munford Middle School, you will need to have the information with you that is listed below to register at MMS.
(If you were a 5th grader last year at Munford Elementary or Drummonds Elementary and you finished the school year there, all you have to do is come in and register on the first day of school)

1. 2 forms of proof of residency
that indicate physical address (not p.o. box)
A. Mortgage document/property deed
B. Property tax record
C. Copy of lease
D. Current utility bill(not phone)

2. copy of last report card
3. copy of shot record
4. copy of social security card
5. copy of birth certificate
6. proof of custody if not living with both natural parents
7. copy of discipline record
