Beginning in 2008-09 school year,
all 7th and 9th grade students will need a well child exam/EPSDT screen to participate/try-out for athletic teams. Unfortunately, this screening/physical cannot be administered at the school as we do with the regular sports physicals at this time. We have the new form in the office.
If your child is going to be a 7th grade student in 2008-09, they will have to have the well child exam/EPSDT examination before trying out for a sports team at MMS.
If you do not have this exam and you are going to be a 7th grader at MMS in 2008-09, you will not be allowed to participate in athletics.
This is a state requirement that goes into effect beginning May 1, 2008 for all students entering 7th and 9th grades in Tennessee.

We encourage all students who will be in 7th grade in 2008-09 to get this special exam in the summer from their physician. If you wait until school starts in August, you may not be able to participate in football, cheerleading, or volleyball.

This is a link to the general website. As soon as we have completed teacher training we will begin using the system. We hope everything will be in place by the beginning of the 4th nine weeks.

Cars should not be parking off of the road down from the school to pick up students who are walking home. All cars should stay in line and pick up students in back of the school. Cars parking off the road create a safety concern for walkers and traffic.

Please tell your students that walk home in the afternoon to stay on the sidewalk and not to go across the hill in the wooded area.

PHYSICALS

For students who will be in 8th and 6th
grade during the 2008-09 school year.

If you are getting a physical from your own doctor, the forms must be signed by the parent. The parent must fill out and sign the pre-existing condition page (first page) and the permission to treat form (last page). You can access these forms by going to www.tssaa.org and clicking on "forms" at the top of the page. Then click on "physicals" at the top of the next page.

Students must have a current physical before they are allowed to try-out or practice with a Munford Middle School Team.
This physical needs to be dated after May 1, 2008. No exceptions. Camp participants do not have to have a physical, only cheerleaders for cheer camp.

For students who will be in 7th grade during the 2008-09 school year, you will need to pick up a form from the office.
We hope to have a web link in the future so you can download a form. 7th grade students have to have a child wellness exam/EPSDT exam before being allowed to try-out or participate in athletics.
