· Immunization record transferred to a Tennessee

· Withdrawal form or last report card from previous
school
· Twp proofs of residence
Are you enrolling from a private school?
· Withdrawal form or last report card from previous
school
· Twp proofs of residence
· MES will test your student to determine grade
placement
What are acceptable “proofs of residence?”
· Property tax record
· Mortgage document or property deed
· Apartment or home lease
· Current utility bill (water, phone, electric)
· Automobile registration
· Voter registration
Special Note…
If the student’s name is different from the parent’s,
a copy of marriage license, birth certificate,
or custody papers will be required.
