The TSSAA Forms are now
available online. You will need Adobe Acrobat to read these files. A physician will be at Crestview in May 2010 to perform physicals for students who are interested in playing in athletic sports in the 2010-2011 academic year. The cost of this physical: $10 cash only. Parents must fill out the medical history portion and sign before a physical will be given.
