The TSSAA Forms are now available online. You will need Adobe Acrobat to read these files. A physician will be at Crestview May 12 to perform physicals for students who are interested in playing in athletic sports in the 2009-2010 academic year. The cost of this physical: $10 cash only. Parents must fill out the medical history portion and sign before a physical will be given.
