How do I obtain a Username and Password? 1. Please make sure that you are a legal guardian, biological, or custodial parent. Open, download, print, read, and sign all forms in the Application Packet. You may print it from clicking the hyperlink, or there will be copies in the front office at CMS. You are verifying that you agree to the Acceptable Use Policy and to the Terms of Use, which you may read from the K12 Planet website. Just click the Terms of Use link at the bottom of the web page.
4.No account information will be mailed or sent home with students. Someone from Crestview Middle will contact you when your account is ready for use- usually within 24 - 48 hours. Due to a high amount of applications that have been submitted, it may take more than 48 hours to process your application.
