How do I obtain a Username and Password?
1. Please make sure that you are a legal guardian, biological, or
custodial parent. Open, download, print, read, and sign all forms in the Application Packet. You may print it from clicking the hyperlink, or there will be
copies in the front office at CMS. You are verifying that you agree to the Acceptable Use Policy
and to the Terms of Use, which you may read from the K12
Planet website. Just click the Terms of Use link at the
bottom of the web page.
2. Feel free to bring the application to the Crestview Middle front office or you may mail the application to:

3. All accounts are
generated from the school's student management system. If there is a problem with your account, a CMS staff member will contact you. If you include an active e-mail address in your application, you will be contacted to call the school regarding your account at the time Mr. Mooney attempts to process your account. You will either receive an e-mail confirming your account approval, on hold, or denied e-mail due to reasons listed in the e-mail. |
4.No account information will be mailed or sent home with students.
Someone from Crestview Middle will contact you
when your account is ready for use-
usually within 24 - 48 hours. Due to a high amount of applications that have been submitted, it may take more than 48 hours to process your application.
Take your signed forms to the front office between 7:00 AM and 4:00 PM Monday - Friday or mail them to CMS at the address above.
Copy of your photo ID is required to receive an account.
All applications and photo ID are kept in a secured location.

of K12 Planet? Please ask for assistance in the front office. We offer you tutoring in the web site after your application has been approved. You may visit one of the computer labs or the library daily from 7:00am to 4:30pm. You

**Note: Factors beyond the K12 Planet Administrator's control may cause data to not be uploaded.
(Internet downtime, hardware failure, or a problem with K12 Planet services is possible.)
