State Birth Certificate
Social Security Card
TN Immunization Form (Green Card)
TN Medical Entry Form (Blue Card)
Physical Form (Kindergarten ONLY)
2 Proofs of 911 Address from the following list: Property tax records which indicate location on home
Mortgage documents or property of deed
Apartment or home lease
Current utility bill showing residence address
Automobile registration
Voter's Precinct Identification Proof of Grade Placement Report Card
Withdrawal Form from Previous School
