Individual Growth Plan

INDIVIDUAL GROWTH PLANS (IGP)

All learners need an IGP (Individual Growth Plan).

o This should be revised 4 times per year.

o Send home an updated copy with each report card.

A permanent copy will be placed in the Cumulative Files (Permanent records kept in the school volt) at the end of the year.

 

STRENGTHS & WEAKNESSES: With learner and/or parent input, list the learner’s academic strengths and weaknesses. Use records from the previous year, TCAP scores, initial STAR test results, previous IGP, etc…

 

LEARNING STYLE: Use a learning style profile to discover the learning style preferences for each learner. For younger learners, ask parents to complete surveys about their child. Different profiles are available in this Teacher’s Handbook/Notebook. 

 

PERSONAL GROWTH: Learners may want to become

· Peer Tutor before school or during dismissal

· Student Counsel

· Beta Club member (6-8th grade)

· TCAP Boys Club

· Band or Choir Member

· 5th Grade Care Helper

· 6th Grade Clothes Closet

· PreK Helpers (7-8th grade)

· Mentor

 

GOALS: Decide the greatest area of need in each of the core subjects listed on the form. (Personal, Reading, English, Creative Writing, and Math) List a simple and precise goal for each. Writing goals are the referring to the writing process and writing sentences, paragraphs, narratives, etc… rather than handwriting.

 

RECOMMENDATIONS: Learners below proficient could be recommended for more time in the Success Maker Lab. Non-readers could be recommended for time with Mrs. Coop.

 

EVALUATION: During each evaluation, consider the goals for each area of study. If the goal has been mastered then simply write mastered and then record a new goal. If the goal has not been mastered and should be continued then simply write continued. If the goal needs to be revised to meet the current concerns and goals then simply write revise. Then rewrite the goal. 

· Mastered – write a new goal

· Continued – write nothing

Revised – rewrite and revise the goal written to meet the current needs

 

STAR RESULTS: After the administration of each STAR test, list the test results for Reading and Math at the bottom of this form by recording the grade level score.
